Consumer Health Data Privacy Policy

This Consumer Health Data Privacy Policy describes how New Tides Health LLC collects, uses, shares, and protects “consumer health data” — information that is linked or reasonably linkable to you and that identifies your past, present, or future physical or mental health status. It supplements our general Privacy Policy and applies nationwide as our strict standard.

1. Categories of consumer health data we collect

• Health conditions, symptoms, and treatment information you provide or we generate.
• Medications, allergies, and health history.
• Measurements such as height and weight; lab orders and results.
• Information that identifies you as seeking health care services.

2. Sources

Directly from you (intake, visits, messages); from your treating providers; and from the laboratory and pharmacy involved in your care.

3. How and why we use it

To evaluate you, provide and coordinate care, prescribe and fulfill treatment, communicate with you, and meet legal and safety obligations — only for the purposes you have consented to.

4. With whom we share it

Only with your treating providers, the dispensing pharmacy, the laboratory, and service providers bound by written data-protection agreements, in each case as needed to provide your care and with your consent. We share consumer health data only with your separate consent or as necessary to provide a product or service you requested.

5. We do not sell consumer health data

We do not sell your consumer health data, and we will not do so. We do not collect or use consumer health data except with your consent for a specified purpose or as necessary to provide services you request.

6. Your rights

• Right to confirm whether we are collecting, sharing, or selling your consumer health data, and to access it.
• Right to withdraw consent to our collection and sharing of your consumer health data.
• Right to have your consumer health data deleted.

To exercise these rights, email privacy@newtides.com. We will verify your request and respond within the time required by law. If we deny a request, you may appeal by replying to our decision.

7. Data security and retention

We protect consumer health data with the safeguards described in our Privacy Policy and retain it only as long as necessary or as required by law.
