Telehealth Informed Consent

I consent to receive health care services from New Tides Health LLC providers using telehealth (secure messaging, questionnaires, and/or video). I understand:

• Telehealth allows a provider to evaluate and treat me remotely; it has benefits (convenience, access) and limitations (the provider cannot physically examine me, and technology may fail).
• A provider will use independent medical judgment and may decline to prescribe if treatment is not appropriate or safe for me.
• Telehealth is not for emergencies; in an emergency I will call 911.
• My information is protected as described in the Privacy Policy and Consumer Health Data Privacy Policy.
• I may withdraw this consent at any time, which will end future telehealth services.

I have had the opportunity to ask questions and I consent to telehealth care. You acknowledge this consent during account setup and intake; a timestamped record is kept on your account.
